In collaboration with the Dr. Ana McKee, Executive Vice President and Chief Medical Officer at The Joint Commission, Dr. Kevin Weiss will share the data specific to care transitions from the recent CLER National Report of Findings 2016 and engage the collaborator and the community in a robust discussion about the challenges and opportunities presented in the findings.
The speakers will discuss key themes and data from the National Report of Findings 2016 relating to care transitions. He will highlight findings in four major areas: alignment of priorities; inpatient transition processes; change-of-duty transitions; and resident and fellow engagement in developing and implementing strategies to improve care transitions. He will also expand upon these areas by including challenges and opportunities for improvement.
